Lunch Break from 12 PM-1 PM

A Note for Parents: bring all insurance cards and your child's immunization record. VFC-qualified children will not be turned away due to their inability to pay! If you bring someone else's child to the clinic, you MUST have a signed permission slip from the parent or guardian. This permission slip must state: who is bringing the child, what vaccines they need, the child's date of birth, and a parent or guardian’s phone number.
